INTAKE INFORMATION

Walk-In Intakes are Mondays, Tuesdays, Thursdays at 10:00 AM and Wednesdays at 4:00 PM

Please bring a Photo ID, Social Security Card, Insurance Card (if applicable) and Proof of Address.
